The integration of bioceramic materials in endodontics has revolutionized treatment protocols, offering improved biocompatibility, superior sealing ability, and bioactivity. These materials have been widely adopted for vital pulp therapy, root canal obturation, perforation repair and apical plug. Their ability to induce tissue regeneration and form a hermetic seal has significantly enhanced treatment outcomes. However, despite these advantages, concerns remain regarding their handling characteristics, long setting times, and retreatability. While clinical studies highlight their success in improving periapical healing and reducing microleakage, some practitioners argue that conventional materials like resin-based and zinc oxide-eugenol sealers still provide predictable outcomes. Additionally, the high cost of bioceramic materials and limited long-term clinical data raise questions about their widespread adoption.
Recent advancements have focused on improving their mechanical properties, antimicrobial efficacy, and ease of use, addressing previous limitations. As research continues, the debate persists. This lecture will explore the current evidence, weighing the benefits and challenges, to determine whether bioceramic materials truly redefine endodontic treatment or if their impact is exaggerated.

Cone Beam Computed Tomography (CBCT) is an imaging method that has been in clinical use for 25 years and can successfully produce the cross-sectional images needed in implant surgery. Initially, CBCT was primarily used in the pre-surgical planning process and in the evaluation of complications that could arise post-operatively. Today, the scope of this device has expanded to include the field of endodontics.
Since CBCT is a tomographic system that uses ionizing radiation, the formation of artifacts is a significant concern. These artifacts can negatively impact image quality and hinder the accurate assessment of teeth being examined from an endodontic perspective.
However, properly configuring the settings of CBCT can help reduce artifacts and obtain clearer images. This information is crucial for the successful diagnosis of post-operative endodontic treatment issues and for planning the treatment process. Nonetheless, the role of CBCT in the diagnostic process can have both positive and negative effects.
In particular, artifacts and technical issues in CBCT images can complicate the creation of an accurate treatment plan and lead to unnecessary concerns. Therefore, when using CBCT, factors that may affect image quality should be considered, and appropriate technical adjustments should be made. These adjustments are essential for the effective use of CBCT devices and their ability to adapt to the changing conditions of today.
In conclusion, CBCT is an important imaging tool in the field of endodontics. However, it can also carry potential risks if not used case-specifically. Therefore, technical knowledge and experience are required for the effective and safe use of CBCT.

Periapical extrusion accidents in endodontics are the inadvertent displacement of materials beyond the root apex during root canal therapy, which can lead to consequences like as inflammation, infection, cytotoxicity and genotoxicity. It can lead to more serious problems, especially in the case of penetration into the surrounding anatomical structures (eg. the inferior alveolar neurovascular bundles, mental foramen and nerve, and maxillary sinus). These accidents can be caused by factors like excessive instrumentation, overfilling of root canals, improper irrigation, inaccurate working length determination, and inappropriate use of files. When these problems are encountered, the first emergency intervention for focusing to minimize the harm and long-term follow-up of the patient are of great importance. In some cases, advanced surgical interventions may be required. By following correct endodontic protocols, extrusion accidents can be minimized or, if they do occur, successful results can be achieved by performing appropriate interventions.

Vital pulp treatment has the potential to significantly extend the life of a tooth. However, preserving pulp vitality following exposures or caries remains as a real challenge where healing is highly dependent on the inflammation degree. The balance between inflammation and regeneration is crucial in determining the overall pulp healing capacity within an inextensible environment.
Tricalcium silicates are now considered as the materials of choice for vital pulp therapy due to their chemical, physical and bioactive properties. In addition to the hermetic sealing they provide once applied onto the pulp, newly reported data demonstrated that they have an anti-inflammatory potential which creates the adequate conditions for dentin-pulp regeneration and successful clinical outcome.

Micro Endodontic Surgery is a crucial procedure aimed at resolving persistent infection or pathology at the apex of a tooth’s root after conventional root canal therapy has failed. Traditionally, this type of surgery required invasive techniques involving the use of rotary instruments and hand pieces to remove bone and access the apical region of the root. However, over the past decade, advancements in technology have significantly improved the precision, efficiency, and patient comfort of these procedures.
One such advancement is the introduction of the Piezotome, a device that uses ultrasonic technology to facilitate bone cutting with exceptional precision. The Piezotome is now recognized as a transformative tool in Micro Endodontic Surgery, offering distinct advantages over conventional surgical methods. Its operation is based on piezoelectricity, in which an electric current generates mechanical oscillations, which are transferred to an insert specially designed to cut the bone in a controlled manner. This prevents excessive bone damage and reduces the risk of intraoperative complications such as bleeding. During bone cutting through an air-water cavitation effect, they maintain a blood-free field. The benefits of the Piezotome include reduced thermal damage to tissue, as it operates at lower temperatures, minimising the risk of complications associated with thermal necrosis. In addition, it facilitates access to complex root anatomies, making surgery less invasive - so tricky when it’s closed to non-odontoid anatomy like sinus or alveolar nerve - and more comfortable for patients, with less pain and swelling after the procedure. Clinical studies show that patients who have undergone Piezotome-assisted surgery experience less pain and faster healing, with improved bone preservation. In addition, the Piezotome improves visibility and control during surgery, enabling greater precision.
In conclusion, the Piezotome represents a major advance in apical endodontic surgery, offering a more precise and less invasive alternative to traditional methods, contributing to better clinical outcomes and increased patient satisfaction.
The mechanical objectives of root canal instrumentation are to shape the canal in a way that allows for effective irrigation and medication delivery while preserving the integrity of the root structure by minimizing unnecessary dentin removal. Additionally, proper instrumentation facilitates optimal obturation. The biological objectives include the removal of infected tissue, debris, and bacteria from the root canal system while preventing debris extrusion beyond the apical foramen and ensuring instrumentation remains confined within the root canals.
Case selection based on difficulty assessment is a critical step in root canal treatment. Within the endodontic specialty, it is widely acknowledged that the management of complex cases—such as calcified, curved, and MB2 canals should be entrusted to endodontists or general dentists with significant endodontic experience. However, advancements in technology and metallurgy have simplified endodontic procedures without compromising quality. As a result, less experienced clinicians can now manage more complex cases with greater confidence. A prime example of such simplification is single-file asymmetrical reciprocation.
This lecture will examine the advancements introduced by single-file asymmetrical reciprocation and their influence on case selection based on difficulty assessment. It will also explore how these innovations enable less experienced clinicians, including undergraduate students and newly graduated dentists, to manage more complex cases with greater confidence and efficiency while adhering to the biological and mechanical principles of root canal instrumentation.

He also introduced an approach for the emergency management of teeth with symptomatic irreversible pulpitis without a pulpotomy or pulpectomy, and a novel approach for reproducible successful pulpal anesthesia in mandibular molars presenting with asymptomatic irreversible pulpitis without the need of supplementary anesthesia (intra-ligamentary, intra-osseous or intra-pulpal) after the access cavity is started.

Endodontics and restorative dentistry are essential branches of oral healthcare, working together to preserve and restore tooth health and functionality. Digital innovations in dentistry have transformed diagnostic and treatment methods, improving precision, efficiency, and patient outcomes. Therefore, the integration of digital technologies in endodontics and restorative dentistry has significantly changed how dental professionals diagnose, plan, and apply treatments. Incorporating digital advancements in dentistry includes using computer-aided design and computer-aided manufacturing (CAD/CAM) systems, three-dimensional printing(3D), intraoral scanners, digital radiography, and digital treatment records. Digital dentistry therefore enables the use of computer-assisted devices, methods, and technologies—either fully or partially—for the diagnosis, planning, and treatment of dental cases including endodontically treated teeth. CAD/CAM systems offer several benefits, including the ability to produce chairside restorations in a single visit. They provide high precision and quality, eliminate traditional lab methods, and reduce the need for conventional impressions. Additionally, they allow for occlusion adjustments and improve cross-infection control in the dental lab. CAD/CAM systems have also some drawbacks, including high costs, a learning curve, and the potential for devices to become outdated as technology advances. Scanning can also be affected by longer scan lengths, and factors like blood, saliva, and subgingival areas can interfere with the process. Continued education and hands-on training programs can assist dentists in gaining better skills and knowledge. Most dental professions indicate a positive attitude toward digital dentistry and enthusiasm for learning emerging technologies in the dental field. This presentation will summarize the use of CAD/CAM technology in today’s practice and training of dentistry with endodontically treated teeth.

Pain serves as a defense mechanism; however, it is an unpleasant and undesirable sensation. Dental pain is frequently associated with endodontic treatment, and patients with dental anxiety are most concerned about undergoing an endodontic procedure. Therefore, in addition to eliminating pain, addressing the patient's emotional well-being is essential. Endodontists encounter pain-related issues at every stage of treatment—before, during, and after the procedure. Consequently, endodontists must have a comprehensive understanding of pain causes and apply appropriate management strategies. Pain management may require not only clinical interventions but also pharmacological and psychological approaches. Various challenges can arise in clinical pain management. These include difficulty in identifying the source of pain, the sudden onset of acute pain during treatment despite an initially pain-free condition, postoperative pain, and insufficient pain control due to ineffective local anesthesia. This presentation will address key topics related to clinical practice, including identifying the source of pain, treatment options for emergency pain conditions, postoperative pain management, approaches for patients with anxiety, issues with local anesthesia, and the diagnosis, treatment, and prevention of flare-ups. Additionally, pharmacological approaches to pain control will be discussed, offering practical insights for clinical practice.

Manufacturing processes have significantly improved in the last decade due to the development of new heat treatments of the alloy and/or grinded instruments. These improvements have led to a significant change in the mechanical properties of the instruments, but still the clinical differences between the traditional superplastic alloys and the new martensitic ones have not been adequately clarified and the majority of dentists still use different instruments in a similar way. Therefore, the lecture will explain the clinical advantages provided by heat treating the nickel titanium instruments both in rotary and reciprocating motions. It will show how the dentists can utilize these favorable properties during their shaping procedure, maximizing efficiency and minimizing the risk of iatrogenic errors.
